Manual Therapy Birmingham: Hands-On Treatment by a Physiotherapist

Manual therapy in Birmingham at Prime Physiotherapy Clinic means hands-on treatment from a physiotherapist. They use their hands to move stiff joints and work on the muscles around them. The price is £50 for 45 minutes.

The NHS physiotherapy guide lists manual therapy among physiotherapy treatments. Your physiotherapist chooses it after an assessment and pairs it with exercise and advice.

Chiropractors and osteopaths also work hands-on, but they are separate professions. Our osteopath vs physio guide explains how they differ.

Mobilisation and Manipulation: What Your Physiotherapist Does

Your physiotherapist uses three manual therapy techniques:

  • Mobilisation: slow, rhythmic movements of a stiff joint, within a range you can tolerate, with pressure adjusted as you go. It is the only technique we use on the neck.
  • Manipulation: a small, quick thrust at the end of a joint’s movement. Only when suitable, after health questions, once explained and agreed; never on the neck.
  • Soft tissue techniques and stretching for the muscles around the joint.

What Manual Therapy May Help With

Within physiotherapy, hands-on treatment may help with:

  • back pain and lumbago
  • joint pains, for example in the shoulder, hip or knee
  • arthritic pain
  • muscle spasms
  • minor sports injuries
  • general aches and pains

NICE guidance on low back pain and sciatica says it can be considered only as part of a treatment package that includes exercise. For hip or knee osteoarthritis, only alongside therapeutic exercise, not on its own.

So every plan includes an exercise programme. See also our sciatica guide, neck pain exercises and frozen shoulder guide.

Your First Manual Therapy Appointment

Your first 45 minutes cover a physiotherapy assessment and your first treatment:

  • questions about your symptoms, health and any medicines
  • a hands-on check of how the area moves
  • treatment, if it suits you
  • exercises to take home, on a printed or emailed sheet

Wear comfortable clothes you can move in, such as shorts, a T-shirt and trainers; only the area being treated is uncovered. Say if anything feels sharply painful.

When Manipulation Is Not Suitable, and When to Get Urgent Help

NHS guidance says hands-on treatments like chiropractic and osteopathy may not be safe if you:

  • are on blood thinners, or are more likely to bleed or have a stroke
  • have very weak or seriously damaged bones or joints
  • live with a nerve condition, for example peripheral neuropathy

If one applies, we use gentler techniques or suggest a GP.

Call 999 or go to A&E with back pain and:

  • both legs feeling weak, numb, tingly or painful
  • numb skin around your genitals or bottom
  • bladder or bowel changes, or chest pain
  • it started after a serious accident

Call 999 for stroke signs: face drooping, arm weakness, speech problems.

Get an urgent GP appointment or call NHS 111 for:

  • back pain with feeling hot, cold, shivery or unwell, or severe pain that starts suddenly or worsens quickly
  • sudden or very bad shoulder pain, an arm you cannot move, or a badly swollen or misshapen shoulder

See a GP about neck pain with pins and needles or a cold arm. General information only, not a diagnosis; NHS and NICE pages checked on 19 October 2026.

Frequently Asked Questions

Is manual therapy the same as massage?

Not quite. Massage works mainly on muscles and soft tissue, while manual therapy also moves stiff joints with mobilisation and, only when suitable, manipulation. Your physiotherapist may add soft tissue work and always pairs it with exercises.

Is it the same as chiropractic or osteopathy?

No. Chiropractors and osteopaths also work with their hands, but each is a separate profession with its own regulator. Here, a physiotherapist gives manual therapy as one part of physiotherapy, with exercise, advice and manipulation only when suitable.

Does manual therapy hurt?

It may feel firm or mildly uncomfortable, but it should not be sharply painful. Let your physiotherapist know how it feels at any point: they can ease the pressure, switch to a gentler technique or stop.

Is manipulation safe for me?

We ask health questions first. The NHS says hands-on treatments like chiropractic and osteopathy may be unsafe with blood thinners, very weak bones or joints, or a nerve condition. We manipulate only when suitable and agreed, and never the neck.

Does manual therapy work on its own?

Not according to NICE: with low back pain and sciatica, it should only be part of a treatment package that includes exercise. With hip or knee osteoarthritis, only alongside therapeutic exercise. So your plan always includes exercises.

What does manual therapy cost, and can I book without a GP?

Each 45-minute appointment is £50, your first one included. You pay per visit, with no insurance, packages or prepaid sessions, and you can book without a GP referral. Cancelling with under 24 hours' notice may cost the full fee.

What happens at my first appointment?

In 45 minutes, you have an assessment and your first treatment. Expect health questions, a check of how the area moves, then hands-on treatment if it suits you. Wear comfortable clothes; only the area being treated is uncovered.

How many manual therapy sessions might I need?

There is no fixed number. Your physiotherapist reviews your plan and progress at each visit. They explain the next step, which may be more hands-on treatment or exercises on their own.
