Understanding and Managing Lower Back Pain: A Physiotherapist’s Guide

Lower back pain is one of the most common reasons people visit a physiotherapy clinic. In fact, studies show that approximately 80% of adults will experience significant lower back pain at some point during their lifetime. The good news? Most cases respond exceptionally well to conservative physiotherapy treatment.

What Causes Lower Back Pain?

Understanding the root cause of your pain is the first step toward effective treatment. The most common causes we see in our clinic include:

  • Muscle strain and ligament sprain — Often from lifting heavy objects incorrectly or sudden awkward movements.
  • Poor posture — Prolonged sitting, especially at a desk, places tremendous stress on the lumbar spine.
  • Disc degeneration — The intervertebral discs naturally lose hydration and flexibility with age.
  • Sedentary lifestyle — Weak core muscles fail to provide adequate spinal support.
  • Stress and tension — Psychological stress manifests physically, causing muscle guarding and spasm.

Evidence-Based Treatment Approaches

At Prime Physio, we take a holistic, evidence-based approach to treating lower back pain. Our treatment plans are tailored to each individual patient and may include:

1. Manual Therapy

Hands-on techniques such as joint mobilisation, soft tissue massage, and spinal manipulation can provide immediate pain relief and improve mobility. Our therapists are trained in a variety of manual therapy approaches to find what works best for you.

2. Therapeutic Exercise

A structured exercise programme is the cornerstone of back pain recovery. We prescribe specific exercises to strengthen the core stabilising muscles, improve flexibility, and correct muscle imbalances. Research consistently shows that active rehabilitation produces better long-term outcomes than passive treatments alone.

3. Postural Education

We assess your workplace setup, daily habits, and movement patterns to identify postural issues contributing to your pain. Small changes in how you sit, stand, and move can make a dramatic difference.

“The most effective treatment for chronic lower back pain combines manual therapy with a progressive exercise programme. Patients who stay active and follow their rehabilitation plan consistently achieve the best outcomes.”

Five Exercises You Can Start Today

While we always recommend a professional assessment before starting any exercise programme, these gentle movements are generally safe for most people experiencing mild lower back discomfort:

  1. Pelvic Tilts — Lie on your back with knees bent. Gently flatten your lower back against the floor by tightening your abdominal muscles. Hold for 5 seconds. Repeat 10 times.
  2. Cat-Cow Stretch — On all fours, alternate between arching your back upward (cat) and letting it sag downward (cow). Move slowly and breathe deeply. Repeat 10 times.
  3. Knee-to-Chest Stretch — Lying on your back, pull one knee toward your chest and hold for 20-30 seconds. Alternate legs. Repeat 3 times each side.
  4. Bird-Dog Exercise — From all fours, extend your right arm and left leg simultaneously, keeping your back flat. Hold for 5 seconds. Alternate sides. Repeat 8 times.
  5. Bridge Exercise — Lying on your back with knees bent, lift your hips off the floor until your body forms a straight line from shoulders to knees. Hold for 5 seconds. Repeat 10 times.

When to Seek Professional Help

While mild back pain often resolves within a few weeks, you should consult a physiotherapist if you experience:

  • Pain lasting longer than 4-6 weeks
  • Pain that radiates down your leg (sciatica)
  • Numbness, tingling, or weakness in your legs
  • Pain that wakes you at night
  • Difficulty with bladder or bowel control (seek immediate medical attention)

Don’t let lower back pain limit your life. With the right treatment approach and commitment to your rehabilitation, most patients experience significant improvement within 6-12 weeks. Book a consultation with our team to start your recovery journey today.
